Adult Social Care Commissioning and Placement Details

1. An organisational structure chart for Adult Social Care in your Local Authority. 2. The name, job title, work email addresses and work contact numbers for your teams and staff members responsible for arranging, commissioning, referring, and agreeing placements, in working age and older peoples nursing and residential homes. This will include: a) Executive Roles i.e. Directors/Deputy and Assistant Directors b) Heads of Service/Strategic Roles c) Service Managers d) Social Work Teams e) Referrals/Brokerage Teams 3. The number of placements in Nursing/Residential Care with costs exceeding £2,500 per week/£130,000 per annum. 4. The number of placements from the Local Authority in Nursing and Dual Registered Homes that are joint funded with NHS funding bodies. 5. Details of any frameworks or similar procurement arrangements operated by the Local Authority in relation to Adult Nursing and Residential placements.
